RESUMO

O objetivo deste estudo foi relatar a experiência de criação e implementação de um programa para promoção da saúde mental de adolescentes escolares. Trata-se de um relato de experiência obtido a partir da imersão no contexto escolar, no período de 2019 a 2021, para realização de inquérito epidemiológico, seguido da elaboração de um programa para capacitação de familiares de adolescentes e trabalhadores escolares sobre saúde mental de adolescentes. A pesquisa ocorreu em escolas públicas de um município baiano. Todas as experiências obtidas a partir de observação participante foram registradas em um diário de campo. O sofrimento psíquico percebido entre adolescentes tem sido uma questão preocupante, pois afeta a qualidade de vida desse grupo. Ademais, notou-se distanciamento dos serviços de saúde do ambiente escolar, além da dificuldade de familiares e trabalhadores na abordagem do problema. Nesse contexto, propôs-se o programa intersetorial "Adolescer com saúde", com o objetivo de capacitar familiares e trabalhadores escolares para lidar com tal problemática. O programa favoreceu a aproximação entre profissionais de saúde, da educação e familiares, fomentando a corresponsabilização pela promoção da saúde do adolescente.


This experience report presents the creation and implementation of a health program aimed at the mental health of adolescents. After conduction of an epidemiological survey in public schools from a municipality in Bahia, from 2019 to 2021, a program to train family members and school workers on adolescent mental health was elaborated. All experiences obtained by means of participant observation were recorded in a field diary. Perceived psychic suffering among adolescents has been a matter of concern, as it affects their quality of life. Moreover, we observed an absence of health services from the school environment, as well as difficulties by family members and workers in addressing the problem. Given this scenario, the intersectoral program "Adolescer com Saúde" [Adolescence and Health] was proposed to educate family members and school workers on how to tackle the issue. The program fostered a rapprochement between health professionals, educators and family members, encouraging co-responsibility in promoting adolescent health.


El objetivo de este estudio fue reportar la experiencia de creación e implementación de un programa de promoción de salud mental de los adolescentes en la escuela. Este es un reporte de experiencia obtenido de la inmersión en el contexto escolar, de 2019 a 2021, para la realización de una encuesta epidemiológica, seguida de la elaboración de un programa de formación en salud mental de los adolescentes destinado a familiares de adolescentes y a trabajadores escolares. La investigación se llevó a cabo en escuelas públicas de un municipio de Bahía, Brasil. Se registraron todas las experiencias obtenidas de la observación participante en un diario de campo. El sufrimiento psíquico percibido entre los adolescentes ha sido motivo de preocupación, ya que afecta a la calidad de vida de este grupo. Se constató que los servicios de salud estaban alejados del ámbito escolar y que había dificultad de los familiares y trabajadores para abordar el problema. En este contexto, se planteó el programa intersectorial "Adolescencia con salud", con el objetivo de capacitar a familiares y trabajadores escolares para enfrentar esta problemática. El programa favoreció el acercamiento entre los profesionales de la salud, los de la educación y los familiares, incentivando la corresponsabilidad en la promoción de la salud de los adolescentes.

International deployment of remote monitoring and virtual care (RMVC) technologies would efficiently harness their positive impact on outcomes. Since Canada and the United Kingdom have similar populations, health care systems, and digital health landscapes, transferring digital health innovations between them should be relatively straightforward. Yet examples of successful attempts are scarce. In a workshop, we identified 6 differences that may complicate RMVC transfer between Canada and the United Kingdom and provided recommendations for addressing them. These key differences include (1) minority groups, (2) physical geography, (3) clinical pathways, (4) value propositions, (5) governmental priorities and support for digital innovation, and (6) regulatory pathways. We detail 4 broad recommendations to plan for sustainability, including the need to formally consider how highlighted country-specific recommendations may impact RMVC and contingency planning to overcome challenges; the need to map which pathways are available as an innovator to support cross-country transfer; the need to report on and apply learnings from regulatory barriers and facilitators so that everyone may benefit; and the need to explore existing guidance to successfully transfer digital health solutions while developing further guidance (eg, extending the nonadoption, abandonment, scale-up, spread, sustainability framework for cross-country transfer). Finally, we present an ecosystem readiness checklist. Considering these recommendations will contribute to successful international deployment and an increased positive impact of RMVC technologies. Future directions should consider characterizing additional complexities associated with global transfer.

Oxalate nephropathy is a rare cause of kidney failure. Roux-en-Y gastric bypass surgery is a technique used for surgical treatment of obesity as well as for the treatment of gastric carcinoma. We report the case of a 46-year-old male who was admitted to the nephrology department due to kidney dysfunction eight months after bariatric surgery.

Calcifying organisms are considered as threatened by ocean acidification, because of their calcium carbonate skeleton. This study investigated if a calcareous sponge could synthesize its skeleton (i.e. spicules) under ocean-acidification conditions. Sponge cell aggregates that have the potential to develop into a functional sponge, called primmorphs, were submitted to a 5-day experiment, with two treatments: control (pH 8.1) and acidified conditions (pH 7.6). Primmorphs of the calcareous sponge Paraleucilla magna were able to synthesize a skeleton, even under low pH, and to develop into functional sponges. The spicules had the same shape in both conditions, although the spicules synthesized in low pH were slightly thinner than those in the control. These results suggest that P. magna may be able to survive near-future ocean-acidification conditions.

Sponges thrive in the deep, dark and nutrient-depleted ocean and may rely on microbial symbionts for carbon acquisition and energy generation. However, these symbiotic relationships remain largely unexplored. In this study, we analyze the microbiome of deep-sea sponges and show that ammonia-oxidizing archaea (AOA) of the family Nitrosopumilaceae make up at least 75% of the microbial communities of the sponges Aphrocallistes sp., Farrea sp. and Paratimea sp.. Given the known autotrophic metabolism of AOAs, this implies that these sponge holobionts can have the capacity for primary production in the deep-sea. We also show that specific AOA lineages are highly specific towards their hosts, hinting towards an unprecedent vertical transmission of these symbionts in deep-sea sponges. Our results show that the ecology and evolution of symbiotic relationships in deep-sea sponge is distinct from that of their shallow-water counterparts.

Coronavirus disease 2019 (COVID-19) has been associated with acute kidney injury in kidney transplant recipients by several mechanisms. The authors report a case of acute kidney allograft dysfunction in a 48-year-old patient who presented in the emergency room with anasarca and nephrotic syndrome close after mild COVID-19 and no other clinical condition. Histopathology of the allograft biopsy revealed two distinct and simultaneous kidney lesions, collapsing glomerulopathy and thrombotic microangiopathy. Renal function persistently deteriorated, and definitive dialysis was initiated. After excluding other plausible causes for the findings, this case strengthens the hypothesis that the kidney allograft is also a target of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2).

INTRODUCTION: Living donor kidney transplantation (LDKT) is accepted as first-line treatment for patients with end-stage kidney disease with advantages over deceased donor kidney transplantation (DDKT). Still, how the known detrimental effect of HLA mismatch (MM) may hamper these advantages remains unsettled. We sought to determine the effect of the degree of HLA MM, separately in deceased and living donor renal allograft outcomes. METHODS: We evaluated all adults submitted to LDKT and DDKT at our center between 2006 and 2018. Their HLA MM was classified according to the British Society of Transplantation system in low mismatch (LM) (level 1-2) and high mismatch (HM) (level 3-4). Acute rejection (AR) and global or censored graft survival were the outcomes of interest. Recipients were followed up from transplant until death, graft failure or the end of 2020.  Results: One thousand sixty-eight kidney transplant recipients were analyzed, 815 (76%) received a DDKT whereas 253 (24%) received an LDKT. From those submitted to DDKT, 95 (12%) had an LM and 720 (88%) had an HM, whereas in LDKT 32 (13%) had an LM and 221 (87%) had an HM. The AR at one year was 9% in the full cohort. Significant risk factors for AR were HM DDKT (OR:2.3, P=0.047) or HM LDKT (OR:5.6, P=0.003) (LM DDKT as reference), calculated panel-reactive antibody (cPRA) ≥5% (OR:1.9, P=0.040) and delayed graft function (DGF), (OR:3.2, P<0.001). Censored graft survival (CGS) at five years was 96% in the full cohort. Independent predictors for censored graft failure (CGF) were HM LDKT (HR:0.2, P=0.046) (LM DDKT as reference), AR (HR:2.7, P=0.008) and DGF (HR:2.2, P=0.017). Global graft survival (GGS) at five years was 91% in the full cohort. Independent predictors for global graft failure (GGF) were HM LDKT (HR:0.2, P=0.042) (LM DDKT as reference), recipient age (HR:1.8, P<0.001) and DGF (HR:1.8, P=0.006). No AR, CGF or GGF episodes were observed in the LM LDKT group. CONCLUSIONS: In our cohort, the level of HLA MM increased the risk of AR independently of donor type. Considering short graft survival, our results support the advantage of living donor vs deceased donor even with an increased HLA MM. However, its effect on long-term graft survival remains to be settled, emphasizing the need for further studies on this matter.

Sponges-associated microorganisms play important roles in their health and ecology; consequently, they may be crucial in the successful adaptation of exotic species to novel environments. However, few studies have focused on the microbial diversity of exotic sponges, especially those with calcium carbonate spicules (class Calcarea). Therefore, this is the first in situ characterization of the microbiota of the exotic calcareous sponges Sycettusa hastifera and Paraleucilla magna. Our results suggest that S. hastifera has a more stable microbiota than P. magna, as there were no differences in its beta diversity among sampling sites. Conversely, P. magna showed significant differences in its microbial communities, perhaps related to its adhesion to artificial substrate and/or shellfish mariculture activities. Each sponge species presented a single dominant proteobacterial OTU potentially active in the nitrogen cycle, which could help sponge detoxification, especially in polluted areas where exotic species usually establish. Our results show the importance of assessing the microbial diversity to unveil host-microorganism relationships and suggest that these associated nitrogen-cycling microorganisms could favor the success of exotic sponges in new environments.

Abstract Introduction: Kidney transplant (KT) recipients have a high risk for adverse outcomes from infections, such as COVID-19. Methods: We have retrospectively reviewed all KT recipients with documented COVID-19 between March 1, 2020, and March 15, 2021, and analyzed patients' characteristics, clinical course, treatment, and outcomes. Results: We identified 123 patients, 72% were male, with a mean age of 54.5±13.0 years. Twenty percent were asymptomatic, 7% had a nosocomial transmission, and 36% of the remainder required hospitalization. Almost all admitted patients received oxygen, 30% required invasive mechanical ventilation (IMV), more than a half had acute kidney injury, with 10% requiring dialysis, and 20% died. Incidence was comparable to that of the Portuguese population, but the mortality rate was almost four times higher (SMR of 3.768 (95% CI:1.723-7.154). Higher body mass index (OR 1.275, P=0.001), lower baseline graft function (OR 0.968, P=0.015), and nosocomial transmission (OR 13.836, P=0.019) were associated with oxygen demand, whereas female gender (OR 3.801, P=0.031) and lower baseline kidney graft function (OR 0.955, P=0.005), but not body mass index, were associated with IMV and/or death. Conclusion: Mortality rate in KT patients was higher than in the general population and lower baseline kidney function was the most consistent marker for adverse outcomes.


Resumo Introdução: Os receptores de transplante renal (TR) apresentam um alto risco para desfechos adversos de infecções, tais como a COVID-19. Métodos: Revisamos retrospectivamente todos os receptores de TR com COVID-19 documentada entre 1º de Março de 2020 e 15 de Março de 2021, e analisamos as características, curso clínico, tratamento e desfechos dos pacientes. Resultados: Identificamos 123 pacientes, 72% do sexo masculino, com uma média de idade de 54,5±13,0 anos. Vinte por cento eram assintomáticos, 7% apresentaram transmissão nosocomial, e 36% do restante necessitaram de internação. Quase todos os pacientes internados receberam oxigênio, 30% necessitaram de ventilação mecânica invasiva (VMI), mais da metade apresentou lesão renal aguda, com 10% necessitando de diálise, e 20% foram a óbito. A incidência foi comparável à da população portuguesa, mas a taxa de mortalidade foi quase quatro vezes superior (TMP de 3,768 (IC 95%: 1,723-7,154). Maior índice de massa corporal (OR 1,275; P=0,001), menor função do enxerto basal (OR 0,968; P=0,015), e transmissão nosocomial (OR 13,836; P=0,019) foram associados à demanda de oxigênio, enquanto sexo feminino (OR 3,801; P=0,031) e menor função do enxerto renal basal (OR 0,955; P=0,005), mas não índice de massa corporal, foram associados à VMI e/ou óbito. Conclusão: A taxa de mortalidade em pacientes com TR foi mais elevada do que na população em geral e a função renal basal mais baixa foi o marcador mais consistente para desfechos adversos.

Introduction Limited information exists concerning the clinical significance of histologically confirmed antibody-mediated rejection (h-AMR) without detectable circulating donor-specific antibodies (DSA). In this study, we compared the outcomes of patients with h-AMR according to DSA status. Methods A total of 80 kidney transplant (KT) recipients who met the 2018 Banff criteria for h-AMR were included. Clinical and immunological characteristics were evaluated, and outcomes were compared according to DSA status after kidney biopsy (KB). Results There were 57 patients who had DSA-positive (+) h-AMR and 23 patients who had DSA-negative (-) h-AMR. Groups had similar baseline characteristics and time between KT and KB. Concerning histopathological diagnoses/Banff scores, DSA+ patients had higher interstitial fibrosis (ci) and tubular atrophy (ct) (ci+ct) scores and lower arterial hyalinosis (ah) scores compared to DSA- patients. Graft survival (GS) was similar for both groups (64% versus 44% at five years and 44% versus 34% at 10 years). Multivariate analysis revealed the time of KB (less than six months after KT or more than six months after KT) to be associated with GS. A stratified analysis was conducted, targeting DSA status according to the time of biopsy. For KB performed less than six months after KT, GS was higher for DSA+ patients at 10 years (66% versus 23%). For KB performed more than six months after KT, DSA- patients had higher GS at 10 years (58% versus 9%). Conclusion Both the timing of AMR diagnosis and DSA status had an impact on AMR outcomes. For patients diagnosed with AMR more than six months after transplantation, GS was worst for those in which circulating DSA were identified. Biopsy specimens from DSA- specimens had higher ct-ci and ah scores.

Anuria suggests complete urinary tract obstruction, acute cortical necrosis, or massive vascular occlusion. We report a case of bilateral renal artery thrombosis in an 87-year-old woman admitted to the emergency department with abdominal pain, diarrhea, and anuria in the last 24 hours. Serum creatinine at admission was 5.87 mg/dl and urea was 100 mg/dl. Computed tomography showed renal artery thrombosis and partial splenic infarction. A conservative approach was performed with anticoagulation with warfarin. The patient recovered renal function and urine output months later.

INTRODUCTION: Kidney transplantation (KT) from living donors has been shown to have multiple benefits compared to those from deceased donors. We sought to compare significant graft outcomes, namely acute rejection (AR), graft function, and survival between transplant recipients who received a kidney from living related donor (LRD) and living unrelated donor (LURD). METHODS: Our cohort comprised 198 donor and recipient pairs undergoing living-donor KT at our center over 10 years. The LRD recipients were compared with LURD recipients according to demographic and clinical characteristics, transplant variables (including immunosuppression), graft function, survival, and AR rate. RESULTS: The estimated glomerular filtration rate (eGFR) was similar in both groups over the follow-up time i.e., 60-65 mL/min (p>0.05 over 10 years). Censored graft survival was similar between LRD and LURD recipients (96.9% vs. 98.0% at five years and 87.8% vs. 79.4% at 10 years, respectively; p=0.837). The LURD recipients had a higher incidence of AR, although LURD recipient status was not an independent risk factor for AR. Multivariate analysis showed that human leukocyte antigen (HLA)-DR mismatch (MM) was an independent predictor of AR (hazard ratio (HR) 2.256, p<0.05). Both HLA-A and HLA-B MM did not affect the AR HR between the groups. CONCLUSION:  Graft function and censored graft survival rates were similar between LURD and LRD KT recipients in our study. The AR was higher in LURD recipients, although the LURD recipient status was not an independent risk factor for AR. The HLA-DR MM was an independent predictor of AR, while HLA-A and HLA-B MM did not affect AR HR between groups of patients.

BACKGROUND: A living donor (LD) kidney transplant is the best therapeutic option for end-stage kidney disease. Potential donors must undergo multiple analyses and the rates of live donation can be as low as 8% to 18%. Here, we report on the live kidney donor program in our unit with emphasis on the reasons why potential donors do not proceed to donation. METHODS: We performed a single-center retrospective study of all potential kidney donors with a first LD appointment at Centro Hospitalar Universitário do Porto between January 2016 and December 2020. RESULTS: In our cohort there were 395 potential donors. From the potential donors who finished assessment, 131 were approved for donation and 239 dropped out. After assessment, 104 (28.1%) recipients received a living kidney transplant, 24 of which received a living kidney transplant through the kidney paired exchange program. The individuals who did not proceed to the surgery (n = 239) had a median age of 46.5 years, 64.4% were female, and 34 pairs were ABO-incompatible. The most frequent donor-recipient relationships were spouses, siblings and parents. The 2 most important causes of dropout were due to medical, surgical or psychological contraindications and the donor's voluntary withdrawal. When we evaluated the variables most related to dropout, they were not because of being a spouse and ABO incompatibility. CONCLUSIONS: When compared to other studies, we showed a relatively higher rate of successful live donations, possibly aided by the presence of cross-over transplantation. Targeted education and support at an earlier stage of the donor assessment process may lead to a better engagement and lower probability of early dropout.

OBJECTIVE: To compare caries prevalence and extent in adults with or without diabetes mellitus (DM) according to different caries detection criteria (WHO, ICDAS, and Nyvad). MATERIALS AND METHODS: A cross-sectional study was carried out including 122 individuals, 44 without DM and 78 with type 2 DM. Trained and calibrated examiners performed a visual-tactile inspection to record coronal and root caries lesions (weighted kappa > 0.7). Caries prevalence and extent were calculated according to the WHO (only cavitated lesions, missing and filled surfaces), ICDAS (all non-cavitated and cavitated lesions, missing and filled surfaces), and Nyvad (only active lesions, non-cavitated and cavitated). For root caries, lesions were classified as active or inactive. RESULTS: A significantly higher overall caries experience (DMF-S) was observed among patients with DM when the WHO (RR = 1.37; 95% CI = 1.09-1.71) and the ICDAS (RR = 1.32; 95% CI = 1.07-1.62) criteria were adopted. No difference between groups was found when the Nyvad criterion was used, although a low study power was observed in this comparison. Estimates for root caries showed a higher prevalence (PR = 2.65; 95% CI = 1.05-6.70) and risk (RR = 6.02, 95% CI = 1.81-20.00) of total D-S among diabetic patients. CONCLUSIONS: DM can predispose individuals to a higher number of root caries lesions, independently of their past caries experience. Missing teeth can overestimate caries extent in individuals with DM. CLINICAL RELEVANCE: Individuals with DM should be monitored for the prevention and control of root caries. It is recommended to splitting missing teeth from the caries estimates in studies involving adults, particularly diabetic ones.

Objetivo: identificar o papel da equipe de enfermagem de um centro cirúrgico quanto à aplicação da segurança do paciente. Material e Método: trata-se de um estudo descritivo de caráter exploratório com abordagem quantitativa, realizado com profissionais da equipe de enfermagem atuantes no centro cirúrgico de uma instituição hospitalar privada, localizada na Serra Gaúcha, no estado do Rio Grande do Sul, Brasil. Os dados foram coletados através de questionários formulados por 25 perguntas, que posteriormente foram armazenados em planilhas no Microsoft® Windows® Excel® 2010 em forma de tabelas para análise estatística descritiva. Resultados: considerando os critérios de inclusão e exclusão, a amostra foi composta por 24 questionários válidos. Os dados analisados evidenciaram predomínio de concordância entre os enfermeiros e os técnicos em enfermagem referente à adesão da segurança do paciente em centro cirúrgico. Conclusão: a pesquisa ressaltou as dificuldades que os profissionais relatam durante sua jornada de trabalho. Tornou-se evidente a insatisfação dos mesmos em relação ao quantitativo dos profissionais de enfermagem, que, de acordo com eles, é um número insuficiente em relação à demanda das necessidades de um processo de cuidar e de segurança do paciente.


Objective: to identify the role of the nursing team in a surgical center regarding the application of patient safety. Material and Method: this is an exploratory descriptive study with a quantitative approach, carried out with professionals of the nursing team of the surgical center of a private institution, located in the Serra Gaúcha, in Rio Grande do Sul state, Brazil. Data were collected through questionnaires formulated by 25 questions, which were later stored in Microsoft® Windows® Excel® 2010 spreadsheets in the form of tables for descriptive statistical analysis. Abstract Results: considering the inclusion and exclusion criteria, the sample consisted of 24 valid questionnaires. The analyzed data showed a high level of agreement between nurses and nursing technicians regarding adherence to patient safety in the operating room. Conclusion: the research highlighted the difficulties that professionals report during their workday. It became evident their dissatisfaction in relation to the number of nursing professionals, which, according to them, is an insufficient number in relation to the demand for the needs of a patient care and safety process.

Heterogeneous photocatalysis is highlighted to treat volatile organic compound (VOC) emission. Then, this work analysed the influence of palladium (Pd) content loaded in TiO2 on n-octane and iso-octane photodegradation. For this, TiO2 was loaded with Pd in different contents: 0.4%, 0.7%, and 1.0%. The samples were characterized, and the photodegradation experiments were conducted by Pd/TiO2/UV process. The characterization analyses showed that the metal presence did not change the catalyst structure or its surface area; however, it reduced the bandgap energy. The photocatalytic results proved that palladium improved n-octane degradation from 62% (pure TiO2) to 92.6% (0.4%Pd/TiO2) and, iso-octane degradation enhanced from 59% (pure TiO2) to 90.6% (0.7%Pd/TiO2); all results were obtained in the space time of 39 s. Therefore, 0.4%Pd/TiO2 and 0.7%Pd/TiO2 showed better oxidation results to degradation n-octane and iso-octane, respectively. The kinetic model of pseudo-first order showed a good fit for the data of both VOCs. Heterogeneous photocatalysis with Pd/TiO2 showed to be an adequate technique to reduce VOCs emission.

INTRODUCTION AND HYPOTHESIS: Labor is a known risk factor for pelvic floor dysfunction (PFD); however, the impact of operative vaginal delivery (OVD), particularly spatulas, remains unclear. The aim of this study was to compare postpartum PFD symptoms in women undergoing spontaneous vaginal delivery (SVD) and those undergoing OVD. METHODS: An observational prospective study (MOODS: Maternal-neonatal Outcomes in Operative Vaginal Delivery) was enrolled at Hospital de Braga from February to October 2018. All singleton term OVD (Thierry spatulas and vacuum extractor) and a convenience SVD sample were recruited, in a 2:1 ratio. To assess PFD symptoms Pelvic Floor Distress Inventory-20 (PFDI-20) was applied at 3, 6, and 12 months postpartum. The questionnaire is divided into three subscales: Urinary (UDI), Colorectal-Anal (CRADI), and Pelvic Organ Prolapse Distress Inventory (POPDI). RESULTS: Of the 304 women recruited, 207 were included, 34.3% with SVD and 65.7% with OVD. Thierry spatulas were used in 53.7% of women undergoing OVD. Frequency of nulliparous (p < 0.001), episiotomy (p < 0.001), neuraxial anesthesia (p < 0.001), postpartum pain (p = 0.001) and occiput-posterior fetal position (p < 0.001) were significantly higher in OVD. Second phase of labor duration was longer in OVD (p = 0.001). At 3 months postpartum, women undergoing OVD and spatula-assisted delivery had higher UDI score, POPDI score, and global score, with no differences at 6 months and 1 year. After controlling for confounding variables, OVD and spatulas were still associated with greater POPDI scores at 3 months postpartum. CONCLUSIONS: Operative vaginal delivery, particularly with spatulas, seems to be associated with a higher prevalence of early PFD symptoms, mainly regarding pelvic organ prolapse.

Objetivo: desvelar vivências de agentes comunitários de saúde no cuidado da pessoa consumidora de drogas. Materiais e métodos: estudo fundamentado na fenomenologia merleau-pontyana, realizado no primeiro semestre de 2018, em um município de médio porte no interior da Bahia, Brasil. Participaram do estudo sete agentes comunitários de saúde, a partir de três encontros de grupo focal. As descrições vivenciais foram gravadas, transcritas e submetidas à técnica analítica da ambiguidade. Resultados: os participantes vivenciam uma condição conflituosa, na qual, ao mesmo tempo que reconhecem a necessidade de assistir as pessoas que fazem o consumo de drogas, são tomados por um sentimento de impotência que dificulta a prática do cuidado. A impotencialidade parece ser motivada por questões amplas e complexas, a exemplo do preconceito ante os consumidores de drogas e a convivência com o "sucateamento" da atenção básica. Conclusões: evidenciou-se que os agentes comunitários de saúde vivenciam desmotivações para o cuidado das pessoas consumidoras de drogas, motivadas por diferentes e complexas questões, que vão desde a associação do consumo à imoralidade, à violência e ao narcotráfico, até a convivência com o "sucateamento" da estratégia saúde da família e a precarização da categoria desses agentes.


Objetivo: dar a conocer las experiencias de los agentes comunitarios de salud en el cuidado de personas consumidoras de drogas. Materiales y métodos: estudio basado en la fenomenología de Merleau-Ponty, realizado en el primer semestre de 2018 en un municipio intermedio al interior del estado de Bahía, Brasil. Siete agentes comunitarios de salud participaron en el estudio, para el cual se desarrollaron tres reuniones bajo la modalidad de grupos focales. Las descripciones experimentales fueron registradas, transcritas y sometidas a la técnica de análisis de ambigüedad. Resultados: los participantes experimentan una condición conflictiva, pues, aunque reconocen la necesidad de ayudar a las personas que usan drogas, al mismo tiempo se sienten abrumados por un sentimiento de impotencia que les dificulta practicar la atención. Este sentimiento parece estar motivado por factores diversos y complejos, como los prejuicios hacia los consumidores de drogas y barreras dentro de la atención primaria en salud. Conclusiones: se evidencia que los agentes comunitarios de salud experimentan desmotivación hacia la atención de personas consumidoras de drogas como consecuencia de factores diversos y complejos, que van desde la asociación del consumo con la inmoralidad, la violencia y el narcotráfico, hasta el debilitamiento de las estrategias de salud para la familia y la precariedad de sus condiciones como agentes de salud.


Objective: To reveal the experiences of community health agents in the care of drug users. Materials and methods: Study based on Merleau-Ponty phenomenology carried out in the first semester of 2018 in a medium-sized municipality in the interior of the state of Bahia, Brazil. Seven community health agents participated in the study, which included three focus group meetings. The experiential descriptions were recorded, transcribed, and submitted to ambiguity analysis. Results: Participants reported experiencing a conflictive condition since they recognize the need to assist drug users while at the same time they are overrun by a feeling of helplessness that makes it difficult for them to practice care. Such feeling seems to be motivated by broad and complex issues, such as prejudices toward drug users and living with the scrapping of primary health care. Conclusions: Community health agents experience demotivation toward drug users' care due to different and complex issues, ranging from the association of drug consumption with immorality, violence and drug trafficking, up to living with the scrapping of drugs in the family health strategy and the precariousness of the community health agent category
